APPLY FOR A SCHOLARSHIP
TO APPLY FOR CONSIDERATION TO RECEIVE A SCHOLARSHIP THE STUDENT MUST BE A SENIOR IN HIGH SCHOOL OR ATTENDING COLLEGE, UNIVERSITY, OR GRADUATE SCHOOL, AND MAINTAINING A 3.0 GRADE POINT AVERAGE at the time of this application.
Applicant is required to download the form, complete and submit by mail to the listed address with the following:
1) Transcript at time of application
2) A solo picture (Headshot) of themselves (no smaller than 2x2 inches)
3) A short biography. The biography should be 2 paragraphs stating your goals, college you will attend or are attending, intended major, future goals, GPA, and interests.
